- Original Birth Certificate, with certified English translation, if applicable.
- Original Divorce Certificate, with certified English translation, if applicable.
- Original Death Certificate of former spouse, with certified English translation, if applicable.
- Parents’ written consent, written and notarized in English, by a Notary Public or Justice of the Peace, if applicable.
